Circumstances:A man part of a group , who had just closed a 6.6kV circuit breaker in a 6.6kV switch room in a generating station, suffered major burns when it exploded almost immediately (about 1 sec.), having failed to interrupt fault current.

How could this accident have been prevented?
Comment:The men were standing just in front of the circuit breaker at the time. The both died as a result of the severe injuries sustained. A 3rd man some yards away from the others also suffered some burns but fortunately survived.
